Client Appreciation Day Registration Form

Please complete this form and click submit. All registrations must be complete by Monday March 5th, to ensure you receive your ticket in the mail. If you register after this date, your ticket will be available for pick up at the Majestic 10. A separate form must be completed for each person (guests, children, partner) attending the event. Please limit the number of additional guests to 4.
